The CAPP Personal Data Form

This form provides the applicant a systematic way to identify areas of personal and professional activity, such as education, experience, etc. and carries a maximum of 72 points.  The candidate submits the form along with the enrollment fee (currently $150 for members and $200 for non-members based on individual member, not organizational membership) to IPI, where the information will be recorded and validated.  Upon acceptance of the form and fee, the applicant is referred to as a "CAPP Candidate."
